Adoptions, Amendments, Adding a Father's Name, Error Corrections or Name Changes:
The only facility authorized to accept corrections or changes (amendments) to a record is the Texas Department of State Health Services, Bureau of Vital Statistics. Via phone, please call, 1-888-963-7111, select Vital Statistics and ask for corresponding area. The standard fee for amendments is $25, plus $11 for a new certificate.
Apostille - Authentication of Documents:
Statutory Documents Section: Documents issued by a statewide officer, including birth certificates, car title histories, etc. You must submit the complete document for authentication. Documents submitted for authentication must be recently issued certified copies. This office can authenticate birth and death certificates issued by city or local registrars. Birth and Death Records Prior to 1903:
The State Archive Facility the Department of State Health Services, Bureau of Vital Statistics stores birth and death records back to 1904. For information on genealogical searches prior to 1904, contact the Genealogy Collection at the State Archives Library. You may reach them by phone at (512) 463-5463. Certificate of Degree of Indian Blood:
The application for Certificate of Degree of Indian Blood (CDIB) must be completed showing your direct lineage to an original enrollee of the Final Choctaw Dawes Rolls. Call 1-800-522-6170 or visit the Choctaw Nation of Oklahoma. All birth certificates submitted as proof of birth must display the state file number.
Delayed (Filings) Records:
If a record is not filed within a year of birth or death with the Local Registrar in the city/county of occurrence then the record must be filed with the Department of State Health Services, Bureau of Vital Statistics. By phone, call 1-888-963-7111, select Vital Statistics and ask for Delayed Records.

Out of State Birth or Death Records:
If you were born outside the state of Texas, or for deaths which occurred outside the state of Texas, you may contact the Bureau of Vital Statistics in the state in which the event occurred. For more information, please visit the National Center for Health Statistics or VitalChek.
VitalChek is the sole vendor authorized by the Austin/Travis County Health and Human Services Department - Office of Vital Records to process online, fax and phone orders for vital records.
