Definition:
A "Credit Card Abuse Affidavit" is a notarized sworn statement attesting that the signature which appears on the questioned document is indeed a forgery, and not authorized by the account holder.
A Credit Card Abuse Affidavit MUST be provided by the account holder before criminal charges can be filed against any suspected criminal!
An affidavit must accompany each forged or counterfeited item. If you have 10 forged checks, you must provide us with 10 affidavits.
