Financial Crimes Unit: Making a Report
If the crime is in progress call 911. If it is an old forgery/credit card abuse, call 311 and make the report over the phone. After the report is made we request that you send us the following information:
(If any of the below information has already been provided please disregard)
Victims:
- Suspect information (i.e. name, date of birth, address, etc.)
- Original check(s) or credit card receipts (you may have to contact the merchant or bank and have that information sent to us.)
- Forgery Affidavit/Credit Card Affidavit
- Bank or credit card statements showing fraudulent charges.
Merchants:
- Have account holder complete Forgery Affidavit.
- Complete the Merchant's Form
- If account holder has already made a police report in Austin, use the offense number given to them.
- If account holder has not made a police report or made one to a Department other than the Austin Police Department, Call 311 and make a police report.
- Mail the completed Merchant's form, the Forgery Affidavit obtained from the account holder and the original check to the Financial Crimes Unit.
These forms must accompany each check submitted for investigation.
- A separate form must be completed for each check presented or credit card transaction conducted.
- Only checks passed within the city limits of Austin will be accepted.
- Any check received without the proper forms will be returned to the sender.
When sending any documents, please be sure to include your case number. Documents received without a case number will be returned.
If at all possible, we prefer that you mail or fax all information to the address/fax number listed below. Because of the volume of work done daily, it is very hard to see victims on a face to face basis. If you must bring information in to our office, please call ahead for an appointed time, so that we can make sure there is ample time to see to your specific needs. Once we have received the above documents, your case will be reviewed to determine if there is sufficient information available to proceed with an investigation. The investigator assigned your case will notify you if further information is required. Your cooperation is appreciated.
